How It Works
Mechanism of Action Ketamine’s therapeutic effects stems from its action on the brain’s NMDA (N-methyl-D-aspartate) receptors, which are crucial in mood regulation and pain perception. By blocking these receptors, ketamine induces a cascade of neurochemical events that enhance the release of glutamate, the primary excitatory neurotransmitter in the brain. Ketamine is a dirty drug from a neurotransmitter perspective; it hits on not only the NMDA receptor sites but also glutamate, dopamine, serotonin, norepinephrine, mu-opoid pain receptors (down regulating pain and inflammation), benzodiazepine receptor sites (positively affecting etoh and benzodiazepine withdrawal and addiction), the Mammalian Target of Rapamycin (MTOR) and Brain Derived Neurotrophic Factor (BDNF). All of this neurotransmitter modulating magic in short, increases synaptic plasticity, which is essential for mood regulation and cognitive function.
Neuroplasticity and Synaptogenesis One of ketamine’s most significant effects is its ability to promote neuroplasticity, the brain’s capacity to form new neural connections. This process, known as synaptogenesis, helps repair and strengthen synapses that may have been damaged by chronic stress, depression, or other mental health conditions. By fostering these new connections, ketamine helps restore normal brain function and improve mental health outcomes.
Rapid Antidepressant Effects Unlike traditional antidepressants, which can take weeks to show effects, ketamine often provides rapid relief from depressive symptoms. Patients may experience significant improvements within hours to days after treatment, making it a valuable option for those in acute distress or with treatment-resistant depression.
Modulation of Brain Circuits Ketamine also modulates several brain circuits involved in mood, cognition, and pain perception. By influencing these pathways, ketamine can alleviate symptoms of depression, anxiety, PTSD, and chronic pain, offering a multifaceted approach to treatment.
Emerging research suggests that ketamine possesses anti-inflammatory properties that may contribute to its therapeutic effects. Inflammation is increasingly recognized as a factor in various mental health conditions, and ketamine’s ability to reduce inflammation in the brain may help improve symptoms.
Clinical Implications The unique mechanism of action and rapid onset of ketamine’s effects make it a promising treatment option for a range of mental health conditions. At Revitalize Ketamine Clinic, our tailored treatment protocols ensure patients receive the most effective and safe dosages, optimizing their therapeutic outcomes.

We go over this whole workbook in its entirety prior to starting a client on their healing journey with us. We view it as paramount in getting the best treatment outcomes. The more avenues we address a problem from, the more likely we are to get capture.
- Day of, in clinic: We always emphasize working with intention, we provide ‘flight instructions’ to prepare our clientele and go over an intention setting guide. After an infusion we have our prescribers sit in the clinic with the client to go over their experience and to begin the integration work.
- Moving Forward: The latest research supports that it is less important what happens during the ketamine treatment and much more important in what is done after the fact with the neuroplasticity. We work closely with community clinicians to collaborate for our shared clients to get the best possible outcomes. For those who do not have a current therapist, we also offer in house integration therapies, we have master’s prepared clinicians who can get people in quickly for integration therapy. Integration therapy should ideally take place 1-2 days after an infusion and take place weekly during an induction series.

How TMS Works
Mechanism of Action
- Magnetic Field Generation: The TMS device generates a magnetic field that is similar in strength to that used in magnetic resonance imaging (MRI).
- Electromagnetic Induction: When the magnetic field is applied to the scalp, it induces an electric current in the brain through a process called electromagnetic induction. This occurs without causing any pain or significant discomfort.
- Targeted Stimulation: The electric current specifically targets the prefrontal cortex, an area of the brain that is often underactive in individuals with depression. This region is crucial for mood regulation, decision-making, and personality expression.
- Neuronal Activation: The induced electric currents depolarize neurons (nerve cells), leading to their activation. This repeated stimulation can enhance the connectivity and communication between neurons in the targeted brain area.
Neurobiological Effects
- Enhanced Synaptic Plasticity: TMS promotes synaptic plasticity, which is the ability of synapses (the connections between neurons) to strengthen or weaken over time in response to increases or decreases in their activity. This is crucial for learning, memory, and recovery from depression.
- Increased Neurotransmitter Release: TMS has been shown to increase the release of neurotransmitters such as serotonin, dopamine, and norepinephrine, which are just some of the neurochemicals that help regulate mood and emotions.
- Improved Blood Flow: TMS can also enhance cerebral blood flow in the targeted brain regions, contributing to improved brain function and overall mental health.
Conditions Treated by TMS
TMS Treatment Protocols
The Procedure
- Preparation: The patient is seated comfortably, and the TMS coil is positioned on their head.
- Mapping: Before starting treatment, a mapping session is conducted to identify the precise location and intensity needed for optimal stimulation.
- Stimulation Sessions: Each TMS session typically lasts between 20 to 40 minutes. The patient remains awake and alert, experiencing only a tapping sensation on the scalp.
- Course of Treatment: A typical course of TMS treatment involves multiple sessions over several weeks, usually five sessions per week for four to six weeks.
TMS Research and Evidence
Clinical Studies on TMS
- Depression: Studies have shown that TMS significantly reduces symptoms in patients who have not responded to traditional antidepressants. One study found that approximately 50% of patients treated with TMS experienced a significant reduction in depressive symptoms.
- Obsessive-Compulsive Disorder (OCD): Research has also shown that TMS can be beneficial for patients with OCD. A study published in the American Journal of Psychiatry reported that patients receiving TMS targeting the supplementary motor area exhibited notable improvements in OCD symptoms compared to a placebo group.
- Post-Traumatic Stress Disorder (PTSD): TMS has shown promise in alleviating symptoms of PTSD. One such study indicated that TMS treatment led to significant reductions in PTSD symptoms, with many patients experiencing improved overall functioning and quality of life.
