Our goal is to create balance in your life both mentally and physically, allowing you to live your life to the fullest with friends and family for years to come.
The staff here at Revitalize Ketamine Clinic (RKC) has been working with traditional treatments and interventions throughout their healthcare careers and has become increasingly passionate about providing groundbreaking, evidenced-based treatments for psychiatric and physical ailments. Physical wellness leads to increased psychiatric wellness. This is an ideal that the entire treatment team adheres to and strongly believes in. With that underlying tenet, our providers here at RKC treat all of our clients holistically by addressing and treating them individually, knowing that everyone responds differently to different interventions. Ketamine-based treatment has a growing body of evidence for a rapid and robust treatment response for a multitude of psychiatric and health conditions, and we’ve truly seen these treatment be life-changing for folks.
That being said, we view the Ketamine treatments we deliver here as a catalyst for change, (nothing should be viewed as a cure-all or panacea) and it is important to incorporate other augmenting strategies and wellness modalities to obtain the best possible outcomes. We utilize strong partnerships in the community to help refer our clientele to the best psychiatric providers, therapists, and other wellness experts that the local region has to offer. We strongly feel that relationships are important and integral to the human experience. If our clients have relationships that are ongoing with other providers, we not only are ok with that, but we encourage it, and we will do our best to collaborate with these providers to provide the best possible care and, thereby, treatment outcomes for our clients and their loved ones. We love our community and want to help everyone to live their very best lives.
Casey has owned and operated Revitalize Ketamine Clinic in some capacity since its inception in April of 2021. He is very excited to be able to provide this life changing treatment in the community where he grew up and who’s health he is very passionate about. Casey, while working in psychiatry in some capacity since 2015 and following the trajectory of Ketamine as a treatment modality since ~2008, became jaded on the traditional psychiatric treatments as they tend to treat symptoms only. Ketamine treatments, especially when viewed as a catalyst for change as we view them in our clinic, and used holistically, have the capacity to allow clients to treat their problems which are causing their symptoms. Casey doesn’t view himself as a healer, he views clients as always being their own best healers, and works to give them the space they need to process through the things that have been keeping them stuck in some cases for decades. Casey is intensely aware that Ketamine treatments, like so many aspects of medicine, can be done right or done not incorrectly but without the proper set and setting that allow for the best treatment outcomes. Casey is a big believer in evidenced based practice and is always following the latest research and incorporating the most recent evidence into our daily practice here at Revitalize Ketamine Clinic. The robust and quality changes that we have seen daily in our clinic have been intensely empowering for Casey to continue to work to expand the availability of this and treatments like it to more and more individuals who could benefit from a reset.
Phillip ‘Casey’ Dubravcic was born in San Diego, California to his Mom Patty Jo Hueth (Bowman), and Phillip John Dubravcic. He lived in ~ 50 houses throughout his childhood, following his very nomadic and loving parents. This frequent movement made it so he always has had a personable demeanor and enjoys meeting and talking with new people. He ended up in Sedona for grade school and came up to Flagstaff for High School, graduating from Flagstaff High School. He was the second graduating class of Coconino Community College’s Nursing program in 2006 delivering the class commencement speech as the class president, then he attained his bachelor’s of science while working at Flagstaff Medical Center (FMC). As a registered nurse at FMC, he worked in the critical care unit, the cardiovascular intensive care unit, and the pre and post-operative areas gaining extensive knowledge on how to care for critically ill and high acuity patients. During this time at FMC, he found his love for psychiatry, by finding much more value in helping to allay the fears and discomfort of critically ill patient’s loved ones and family members by having long conversations and providing emotional support than in mind all of the high tech life-saving equipment including but not limited to the intra-aortic balloon pumps, continuous renal replacement therapies, roto-prone beds, and the oscillating ventilators. While working, developing, and learning, he was also growing his family. He married his wife Jill Dubravcic and they had three fantastic kids together. He graduated with his Psychiatric Mental Health Nurse Practitioner (PMHNP) degree in 2017.
Jill Dubravcic is an exemplary clinician with a strong background in treating a myriad of conditions. As a native Arizonan and a local to Flagstaff since grade school, she has strong ties in the community and a great love for Northern Arizona. She married her Husband and partner Casey Dubravcic in 2008 after dating for 5 years. She has three amazing kiddos born 2010, 2013 and 2014. She enjoys spending her free time traveling, mountain bike riding, jogging, hitting the gym, and hanging out with her kids husband, friends and family. Jill has been a registered nurse since 2005, a family nurse practitioner (FNP) since 2013 graduating from Northern Arizona University, and she attained her psychiatric mental health nurse practitioner post master’s certificate from the University of Arizona. She worked at Flagstaff Medical Center as a registered nurse in high acuity areas treating critically ill patients with consistent success. She has practiced as an FNP at Flagstaff Family Care Clinic and Northern Arizona University. She is passionate about providing the best possible care across the lifespan. She is very knowledgeable on all aspects of health and the many treatment modalities for all psychiatric conditions including but not limited to; treatment-resistant depression, anxiety, panic attacks, post-traumatic stress disorder, bipolar disorder, acute suicidality and substance use disorder. Jill is also the medical director for our clinic and is able to treat complex regional pain syndrome, fibromyalgia, chronic pain, restless leg syndrome, and neuropathic pain. Jill cares deeply about her community and feels that by providing services here at Revitlize Ketamine Clinic that she can make the world a better place, one patient at a time.
Jen Mills- RN
Jen Mills has been in healthcare for over 25 years. She began as an EMT in the rural communities of Cortez and Durango, Colorado, then completed her nursing degree. In 2005 she joined Flagstaff Medical Center in the ED and worked in a variety of departments over the following 16 years. Prior to healthcare, Jen taught bilingual education in Durango and Farmington in elementary and middle school for 5 years.
Jen believes that fulfillment in a job means connecting meaningfully with people, which is why she enjoys nursing and teaching. In health care, she felt it was becoming more difficult to find this connection. So in July 2022, she discovered Revitalize Ketamine Clinic and joined the team. “Being part of a mental health model that really works has been as uplifting for me almost as much as it is for our clients who are experiencing such positive outcomes. I love being able to help a community that has become my home.”
When Jen is not working, she is mountain biking, rock climbing, running, enjoying musical venues downtown, looking for quirky items at thrift stores, decorating her Airbnb, planning trips in her camper, and trying to escape to an island for a yearly ocean adventure because she is allergic to snow.
Susan Ward Flores- FNP
Susan Ward Flores, NP-C, has 30+ years of experience in healthcare. She earned her
bachelor’s of science in nursing at Auburn University in 1992. As an RN, Susan worked in CVICU, Trauma and Medical ICU, as well as in the emergency department. In 2011 she earned her Master’s of Science in Nursing and became certified as a Family Nurse Practitioner.
As an FNP, Susan worked in emergency departments and with the trauma orthopedic group at Flagstaff Medical Center. Now with a focus on improved wellness through lifestyle choices, Susan has joined Revitalize Ketamine Clinic. She brings a wealth of knowledge, a calm, soothing presence, and a great set of clinical knowledge and her own wisdom to the care of our clients here.
Outside of a healthcare career, Susan enjoys the company of her friends and family. She has a husband and two teenage boys. She enjoys an active, adventurous lifestyle, frequently hiking, biking, and rock climbing throughout the Western United States.
Monica Creek- Lead Clinical Receptionist
Monica is the Lead Clinical Receptionist at Revitalize Ketamine Clinic and a 30+ year resident of the Flagstaff community. As a person with the trait of high sensitivity, she is uniquely qualified to be the first voice and face of a place of hope and healing. She brings both compassion and life experience to provide a safe emotional space for those struggling to ‘stay hopeful’ and reach out for help.
During her free time, Monica enjoys stretching (physically, mentally, emotionally), yoga, walking, being with and supporting her loved ones, the PBS Newshour once a week, sleeping (the great equalizer), and loving other people’s dogs. She is seriously pondering the idea of acquiring a new-used dog of her own just as soon as her dog finds her. It is her great privilege to welcome, educate, and bear witness to each person’s transformation during and following their ketamine infusions.

Bevhion Boudica- FNP
Bevhion (pronounced “bay-vin”) Boudica (pronounced “boody-ka” 😉 grew up on the Diné (Navajo) reservation, attended Coconino High School, and is a graduate of the Family Nurse Practitioner program at Northern Arizona University. Ms. Boudica has long desired to work in healthcare in order to provide much-needed primary care services to numerous at-risk and underserved populations within Flagstaff and, more broadly, Coconino County. She started her primary care private practice, Bevhion Boudica Family Healthcare, in the autumn of 2021.
She began working with Casey, Jill, and the rest of the Revitalize team in the autumn of 2022. She is amazed by the efficacy of ketamine treatment and other psychedelic therapies that are experiencing a long-overdue revival! A tribal member of the Diné Nation, Ms. Boudica has been fortunate to take part in numerous peyote ceremonies throughout her life and has experienced firsthand the transformative and healing effects of psychedelics. Now that the scientific community is catching on to what indigenous peoples the world over have known since time immemorial, she looks forward to assisting and supporting you on your path to mind manifestation!
Jenni Boone- RN
Jenni is a Flagstaff native with more than 17 years of experience in nursing, including neonatal, pediatric, and adult critical care. She has been serving our mountain community as a nurse for 12 years, most recently in IV therapy services. Her favorite thing about nursing is getting to know her patients and their diverse stories. Sharing stories is a powerful connection point, and it brings healing to the individual sharing and the person listening.
Jenni’s family includes her husband and three daughters, who love to spend time outdoors. Hiking, swimming, snowboarding, biking, and even simple walks are their favorite ways to spend time. They also love Harry Potter, baking, and traveling. Their favorite spots to travel are usually somewhere sunny and warm, near the water!
Jenni is excited to be joining Revitalize Ketamine because she believes in treating the patient as a whole. We are our own best healers, and that includes healing our minds. She looks forward to hearing your story and working with you to create better health and wellness.
Kristen Moerhle- LAC
My name is Kristen Moehrle, and I am a Licensed Associate Counselor here in Flagstaff. I am new to the Revitalize team and excited to provide in-house therapy sessions! I am a desert native and enjoy hula hooping, roller skating, and creek days with my dog, Sunshine.
As a therapist, I work with individuals who are often experiencing anxiety, depression, and the aftermath of trauma. I find the added tool of ketamine treatments to benefit a client’s openness and readiness to effectively facilitate their own healing processes. I also genuinely enjoy this work. My approach to counseling includes modalities of Self-Compassion, Acceptance and Commitment Therapy, and Attachment work.
As a fellow human, much of what I have learned about others can be summarized in a sentence: everyone is doing the best that they can. In the ever-moving nature of our lives, we are quick to forget or dismiss much of our effort and victory. We can also become quickly overwhelmed when we reach a point of wanting things to be different. The reality is: we have to start where we are and take small steps to get us where we want to be.
As a counselor, I am convinced that you are right where you are supposed to be. I also believe in your ability to learn to trust your own process of figuring things out. Wherever you are, it would be my honor to collaborate with you in the process of uncovering your personal sense of well-being and how to keep moving toward it.
